Ontarians can now download vaccine certificate QR codes from the Ontario website.
The enhanced vaccine certificate contains a SMARTⓇ Health Card QR code that businesses can scan using Verify Ontario app.
As per the news release, the enhanced vaccine certificate is built to the SMART Health Card standard adopted by the federal government in collaboration with provinces and territories. The new vaccine certificate also has a Government of Canada logo to support Ontarians who are travelling internationally.
The scannable QR code-based vaccine certificate is available for download based on an individual’s birth month over a period of three days.
Those born between:
- January and April can download their codes today (October 15),
- May to August can download on October 16
- September to December can download on October 17
All vaccinated Ontarians can download enhanced vaccine certificates starting October 18 at 6:00 a.m.
Ontarians who want their enhanced vaccine certificate emailed or send via post can call the Provincial Vaccine Contact Centre at 1-833-943-3900.

You will need to provide your Health Card number, and date of birth to download the QR code certificate with your full name, date of birth, dates of your vaccinations and the types of vaccines you received. On the website, select ‘proof of vaccination’ and then select ‘Download your enhanced vaccine certificate’ to get the enhanced vaccine certificate.
You can save the electronic version of their vaccine certificate with a QR code to your phone or print a paper copy.
You will need to continue to show a piece of identification with your name and date of birth along with proof of vaccination when visiting select businesses and organizations.
Ontario says businesses must accept both electronic and paper versions.
Ontario has confirmed that the vaccine receipt without a QR code will remain valid and will continue to be accepted even after the enhanced vaccine certificate comes into effect on October 22nd.
