Ontario announced today the requirement of a vaccine passport with QR code and the Verify Ontario app in settings where proof of vaccination is mandated starting January 4th, 2022.
“As we continue to learn more about the Omicron variant, we are taking additional measures to protect our progress as we head into the winter months,” said Christine Elliott, Deputy Premier and Minister of Health. “Getting vaccinated and adhering to public health and workplace safety measures will continue to protect us and our loved ones against COVID and variants, but we won’t hesitate to continue to take swift action to ensure the continued health and safety of all Ontarians.”

The enhanced vaccine certificate is built to the SMART Health Card standard adopted by the federal government in collaboration with provinces and territories. This vaccine certificate also has a Government of Canada logo to support Ontarians who are travelling internationally.
The app is intended to give businesses and organizations where vaccines are mandatory for access, an easy way to confirm the vaccination status of patrons.
Ontario is also enacting measures to combat the rise in the Omicron variant like delaying the lifting of proof of vaccination requirements beyond January 17, 2022.
All Ontarians aged 18 and over can book appointments for COVID-19 vaccine booster dose beginning January 4, 2022.
How to get Vaccine Passport with QR Code
The QR code can be used digitally or by printing a paper copy.
To get the vaccine passport:
- Download online
- Visit their local library to take a printout.
- Call the Provincial Vaccine Contact Centre at 1-833-943-3900 to have their vaccine certificate mailed to them.
- Visit a Service Ontario centre to receive a printed copy.
